You MUST include our REPAIR REQUEST FORM in the box that will contain the unit that needs repair. You will need to print it and fill it in COMPLETELY. Please do not forget to include your return address and phone number information as well as payment and repair information.
 
Repair Request Form - (in Adobe Acrobat Reader .PDF format)
 
You will need Adobe Acrobat Reader to view and print the form. 
 
After downloading Repair Form, open it with Adobe Acrobat Reader. When ready to print in Acrobat, go to [File], [Print], then in the print window, make sure your printer is selected then click [Ok] to print. 
 
Please Note: Shipping costs back to you will be added to the repair billing. We generally use UPS ground, brown label to return your unit back to you. If you wish to get your unit shipped quicker, you may request UPS 2nd Day or Next Service or Fed Ex.
